Job description
Elara Caring in Bloomington, Indiana is seeking a Medical Social Worker (MSW) to assess psychosocial needs and develop care plans for patients in home care settings. You will collaborate with physicians and the care team to ensure safe, well-coordinated discharge planning and continuity of care.
The role requires an active State Social Worker license and a Master’s or Doctoral degree in Social Work from a CSWE-accredited program, with at least one year of healthcare social work experience.
