Job description
Empatia is seeking a compassionate Palliative Care Nurse Practitioner to deliver advanced, home-based palliative care, partnering with the Palliative Medical Director to optimize quality of life for patients in their homes.
You will perform patient assessments, develop care plans, manage treatments, order tests, and document meticulously while supporting families. This role includes evening/weekend on-call and autonomous practice in a national care network.
