Job description
Clinch River Home Healthcare is seeking a Registered Nurse Case Manager to assess patient needs, coordinate care, and ensure clear communication among all parties involved in patient care.
Responsibilities include managing nursing care, home health aide services, and therapies; maintaining confidentiality per HIPAA and Privacy Act; and ensuring collaborative care across the team. A valid RN license and at least one year in home health are required.
