Job description
Chapters Health System in Florida is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care so patients can stay in their home environment. The role requires a Florida RN license and at least 1 year in hospice or hospital settings.
Strong EMR/EHR skills, a valid driver’s license, reliable transportation, and ability to meet the physical demands are essential.
