Job description
Mountain Valley Hospice is seeking a Registered Nurse Case Manager to provide high-quality nursing care in patients' homes, SNFs, and ALFs. You will collaborate with a team of professionals to support patients and families facing serious illness.
Responsibilities include direct nursing care, coordinating with CNAs and other providers, and monitoring patient conditions to communicate changes to the care team. A compassionate, patient-centered approach is essential.
