Job description
Transitions Hospice in Pontiac is seeking an RN Case Manager to serve as the clinical anchor for patients and families, coordinating care and guiding the interdisciplinary team.
You will conduct assessments, build care plans, administer treatments, and ensure accurate documentation while serving as a communication bridge among families, physicians, and community partners. This role requires compassionate, independent work in a home-based hospice model.
