Job description
St. Croix Hospice is seeking an experienced Registered Nurse Case Manager to deliver in-home hospice care in the Wausau, WI area.
You will collaborate with a care team to create personalized plans, lead meaningful end-of-life conversations, and ensure symptom management with empathy and clinical excellence. The role requires an RN license with an accredited nursing degree and at least 1 year of acute care experience; hospice/home health experience is preferred.
