Job description
St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1 holistic in-home care to patients and families, partnering with an interdisciplinary team to develop individualized care plans and guide end-of-life conversations.
Requirements include an accredited nursing degree and 1+ years of acute care experience; hospice/home health experience is preferred. The position offers a full-time schedule with on-call duties and a comprehensive benefits package.
