Hinds LifeCare, a trusted nonprofit in California’s Central Valley, is hiring a Full Time RN Case Manager in Fresno, CA to join our hospice team.
The RN Case Manager provides skilled palliative and end-of-life care in community settings and coordinates an interdisciplinary plan to manage hospice services for patients and families.
Required: California RN licensure and CPR certification; a valid driver’s license and reliable transportation.
