Job description
St. Croix Hospice is seeking a dedicated Registered Nurse Case Manager to provide 1:1, whole-person care in patients' homes. You will coordinate with an integrated care team to develop individualized plans and guide families through complex decisions with empathy and professionalism.
Ideal candidates hold an RN with 1+ year acute care experience; hospice/home health preferred. Requires timely on-call rotations, reliable transportation, and a commitment to compassionate, dignified end-of-life
