Job description
AbsoluteCARE Medical Center & Pharmacy in Philadelphia seeks a Complex Care Nurse Practitioner to join a multidisciplinary team. The role focuses on counseling patients, assessing medical needs, guiding specialty services, and supporting end-of-life programs in an outpatient setting.
You'll manage a chronic-care panel, coordinate with social work, case management, and other professionals, and contribute to transitional care plans, hospital discharges, and home care when needed.
