Harbor Health Services, Inc. is seeking a Complex Care Nurse (RN) for the Harbor Community Health Center in Plymouth, MA.
The role focuses on population health-based and complex care nursing across high-risk patient populations with care coordination, testing, medication management and patient education to promote wellness.
