Job description
Position Summary
nThis position is responsible for providing direct clinical and case management services, including: intake and assessment with new tenants, collaborative development of individual service plans with tenants, crisis intervention, individual counseling and providing therapeutic group activities to help tenants meet their goals. The case manager provides linkages and coordinates resources/referrals. Case managers assist tenants in goal attainment, including but not limited to: maintaining their housing, obtaining and maintaining sustainable income, and achieving health and wellness goals.
nnEssential Duties and Responsibilities
n Case Management Services
nnProvide case management services including intakes, assessments, service planning, staffing, individual counseling, referral, and advocacy.
nProvide crisis intervention in mental health and/or medical emergencies. Provide on-call assistance with after hour and weekend emergencies.
nAssist with applications and advocacy related to public benefits.
nPlan and implement therapeutic group activities.
nWork with tenants to achieve their goals related to housing, income and health.
nRegularly assess and respond to tenants' motivation and readiness using motivational interviewing skills.
nEngage in clinical supervision, in both group and individual formats.
nWork with tenants to address issues in order to prevent eviction.
n n Record Keeping and Data Quality
nnPrepare and maintain current tenant files.
nEnter all services and required information into HMIS and other service and outcome tracking databases as required.
nWork collaboratively with the Program Data Manager to ensure data quality.

nnMaintain required professional knowledge and job skills. Attend and participate in staff development opportunities.

n nnQualifications and Requirements
nEducation: Bachelor's Degree in Social Work or related field. Master's preferred.
nLicensure and/or Certification: LCSW preferred
nJob Experience: 2-3 years of case management experience
nSpecial Knowledge & Skills: Knowledge of issues surrounding homelessness, mental illness and substance use; proficiency in Microsoft office software, good oral and written communication skills, good organizational skills, conflict management and crisis intervention skills; demonstrate clear and professional boundaries; act in a professional manner; Valid Illinois driver's license, proof of insurance and must have own vehicle.
