Job description

StarCare Specialty Health System in Lubbock, TX, seeks a well-organized Continuity of Care (CoC) Service Coordinator to manage enrollment processes for Medicaid Waiver Programs and provide permanency planning for individuals under 22 in ICF facilities. The role also serves as the liaison for residents at the Lubbock SSLC.
