Job description
Bluebonnet Trails Community Services seeks a State Hospital Coordinator of Care to ensure continuity of care for individuals transitioning to/from the state hospital and to move clients to least restrictive settings. The role provides crisis intervention and jail diversion across eight counties, collaborating with jail personnel, courts, and treatment teams.
The coordinator links hospital and community providers, uses electronic health records to monitor indicators, and completes ANSA/CANS/PCRP
