Sage GroupHealthCare Staffing is seeking RN Hospice for a 13-week travel / contractposition in Grand Junction, CO. Accepting both local and traveler candidates.

Pay Rate:Range $2080 to $2120 Weekly Gross pay based on a 40-hour work week (Includes non-taxabletravel/housing and M&I stipend available for qualified/eligible travelers)
MileageReimbursement: $0.50/mile for distancecovered between patient home appointments
Unit: Home or Community
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri with occasional back up for weekend and on-call. Contract offer flexible scheduling
Position Requirements:
Registered Nurse: 1 to 2 years - Required
Hospice Nursing experience: 1 to 2 years – Required
EMR/EHR charting - Required
Must have validDriver’s License and Personal Auto Insurance
Education, Licenseand Certification Requirements:
ASN or an equivalent– Required
BSN or an equivalent– Highly preferred
RN License for State applying or Compact if accepted in the state - Required
BLS Certificationfrom AHA or American Red Cross - Required
Job Description:
Provide Nursing Care for Hospice Patients inprivate homes, nursing homes, group homes or assisted living facilities.
Manage assigned caseload of patients assignedcaseload of patients and lead the interdisciplinary team caring for thosepatients
Provide assessments, care planning, painmanagement and symptom management and all other aspects of hospice nursing
Ensure timely and accurate submission ofpatient documentation and charting in adherence to client policies.
