Job description
Connections Health Solutions in Virginia is seeking a Case Manager to facilitate admission intakes, treatment planning, discharge planning, and coordination with community partners. You will work with an interdisciplinary team to ensure safe, person-centered care for individuals in crisis and to initiate personalized treatment.
A QMHP and knowledge of Virginia's behavioral health system are required, with full vaccination compliance.
