Job description
Connections Health Solutions in Woodbridge, Virginia is searching for a dedicated Case Manager to facilitate admission intakes, treatment planning, and coordinate care for individuals in crisis. The ideal candidate will have a Bachelor’s degree in behavioral health and must be a Qualified Mental Health Professional (QMHP).
