nPosition Data:

nGrade: 6 Status: Non-Exempt

nReports to: Intensive Services Coordinator

nBasic Function or Position Summary:

nProvide Crisis Intervention and follow-up services to Chautauqua County Residents experiencing mental health crisis from 4 pm – 8 am on weekdays and 24 hours on weekends and holidays.

nSalary $18.81/hourly

nSecurity Clearance:Level 1 V,A,C,R

nRequirements

nPosition Responsibilities and Specific Duties:

n· Assist in the provision of crisis intervention services in the community for children and adults including intake, outreach visits, assessment, referral, follow-up, telephone lethality monitoring, and disposition of all persons presenting for Mobile Crisis Services.

n· Assist in the facilitation of stabilization of acute mental health crisis utilizing the least intrusive and restrictive alternative available to maintain safety.

n· Complete follow-up and facilitate linkages to needed community services and supports for both the primary customer and his/her family.

n· Maintain on-call availability by work issued cell phone during scheduled shifts.

n· Data reporting and analysis as a function of service quality improvement.

n· Maintain confidentiality and comply with the code of ethics.

n· Promote the Agency mission.

n· Maintains consistent professional customer service.

n· Job performance incorporates a strength-based, integrated service delivery model while promoting self-sufficiency.

n· Attend scheduled staff meetings, mandatory trainings, in-services, and other functions as required.

n· Provide marketing and/or educational presentations to the community, or training to other agency staff, as assigned by the Family Growth & Development Manager.

n· Participate in the larger team of the agency through committee structures, planning, policy development and volunteerism.

n· Promote positive internal and external relationships.

n· Other duties as assigned by supervisor.

nKey Working Relationships:

nA. Internal: Director of Health and Family Services, Intensive Services Coordinator, Crisis Team Members, and other COI staff

nB. External: Emergency service providers such as police departments, hospital emergency department, hotline services, other mental health providers, and human service professionals.

nSupervisory Scope:This job has no supervisory responsibilities.

nOrganizational Responsibilities:

n• Adhere to all policies, procedures, and protocols.

n• Gathers appropriate documentation and tracks outcomes.

n• Maintain customer files inputted into CAPTAIN

n• Complete required progress reports on time.

n• Participates in organizational committee structures as appropriate.

n• Participates in organizational and divisional management systems.

nKnowledge, Skills Required:

n· Bachelor’s Degree in a human services field.

n· Experience in Crisis Intervention with the Mentally Ill population required.

n· Possess a working knowledge of community services and agencies

n· Has knowledge and skills necessary to modify interventions and care according to the customer’s age.

n· Excellent oral; written communications skills

n· Ability to work as part of a team required.

n· Must be able to demonstrate initiative and resourcefulness; have analytical ability and be able to collect and research information and use sound judgment.

n· Bilingual (Spanish) preferred.

nSpecial Requirements:

n• Must be able to always provide consistent even tempered customer service

n• Must be able to work evenings, nights, weekends and holidays.

n• Valid Driver’s License and reliable transportation

n• Ability to lift up to 50 pounds with or without reasonable accommodation.

n• Completes all OMH requirements.

n• Must maintain a working telephone

n• Works compassionately with a diverse population

n• Experience navigating community programs
