Unit Details

ED holding is expected to be the primary unit and includes patients waiting for a bed, mixed diagnoses, isolation patients, and non-ambulatory patients; it is a fast-paced unit.

ICU patient types include respiratory failure, GI bleeds, DKA, sepsis, complicated postoperative patients, CVAs, flap surgeries, post-arrest patients, ETOH, and IVDA.

Higher-level therapies listed include Rotoprone, manual prone, hypothermia protocol, CRRT, TPA, and flap surgeries; the traveler will not take these patients.

The ICU does not take craniotomy, major trauma, or major cardiothoracic cases.

PCU patient types include respiratory compromise, sepsis, renal failure, bleeding disorders, general medical-surgical patients, CHF, COPD, pneumonia, general cases, and orthopedic cases.

Med-surg assignments include oncology patients who are neutropenic or postoperative, psych patients, and general medical-surgical patients with postoperative needs, COPD, CHF, sepsis, DVT, AKI, dialysis, or COVID; COVID is common on this floor.

Med-surg orthopedic assignments include total joints, postoperative patients, and general medical overflow.

Med-surg palliative assignments include general medical patients with pneumonia, sepsis, CHF, COPD, AKI, DVT, PE, occasional surgical needs, or COVID.

The flex unit consists of PCU and medical-surgical patients.

Rehab patients include those with stroke, spinal cord injury, TBI, multi-trauma, post-COVID conditions, and post-ICU needs; ventilated and tracheostomy patients are also present, with med-surg-level acuity.

Respiratory therapy provides just-in-time training on the rehab unit.

The RN will transport patients throughout the hospital, including all inpatient units, the ED, CT, MRI, nuclear medicine, PACU, and cath lab.

Day-shift ACLS transport RNs typically start getting busy at and never slow down for the rest of the day.

Patient ratios vary by assignment: ICU 1:2 based on acuity; PCU and CCU 1:4 depending on acuity or whether a patient is discharging; medical and surgical 1:5; and rehab uses a 1:8 team model (RN, LPN, Tech).

Alaris IV pumps and Pyxis medication dispensing are used.

Floating Requirements

The assignment is mainly ED-hold floating, supporting GSH 1 Dally/2 River North and South and Float Pool teams.

The clinician may float for partial shifts.

Floating will remain within the clinician’s scope of practice and below.

If a PCU nurse floats to med-surg, the ratio is 1:5.

Weekends will vary depending on the needs of the unit.

The schedule is provided 3 weeks at a time, received around 10 days before the schedule begins.
