Job description
Amedisys Home Health in Tucson, AZ is seeking a Registered Nurse Case Manager for full-time days. You will perform patient assessments, collaborate with the care team to develop and implement a plan of care, and make referrals to other disciplines as indicated to meet each patient’s needs.
The role includes supervising LPNs and HHAs, documenting timely, and communicating patient progress to the clinical manager.
