Location:CHP Dental Center 515 W. 6th St. Jacksonville, FL 32206
Computer Software: EagleSoft, Electronic Oral Health Record and HMS (Health Management System)
Dress Code: Medical Scrubs
Providing a full range of general dental services including comprehensive dental exams, preventive services, restorations, removable complete and partial dentures and extractions within the scope of a general dentist.

Candidate Responsibilities
Providing a full range of general dental services including comprehensive dental exams, preventive services, restorations, removable complete and partial dentures and extractions within the scope of a general dentist.
Experience/Skills Required
5 years’ experience providing a full range of general dental services including comprehensive dental exams, preventive services, restorations, removable complete and partial dentures and extractions within the scope of a general dentist
Educational Requirement
DDS or DMD Degree
Active Florida Dental License
Current/Active DEA
Current CPR Certification
