Job description
BJC HealthCare seeks a Clinical Nurse Diabetes Educator to provide expert diabetes education and care coordination for patients and families. In this PRN role, you will assess learning needs, develop individualized self-management plans, and deliver instruction on insulin use, glucose monitoring, nutrition, and lifestyle changes. You will collaborate with an interdisciplinary team, document care in the EHR, support quality initiatives, and contribute to community and hospital-based diabetes programs in a mission-driven, patient-focused environment.
