Job description
Sanford Health in Minocqua, WI is seeking a Diabetes Care and Education Specialist to educate patients on diabetes self-management, provide nutrition therapy, and coordinate care with a multidisciplinary team. The role includes training on diabetes technologies and maintaining CDCES certification.
You will work in a hospital-based setting with opportunities to lead patient education initiatives and contribute to quality improvement within the Diabetes Self-Management Education program.
