Job description
The Region HR is seeking a Diabetes Educator to provide DSMES to patients and families in an outpatient clinic setting. You will assess needs, develop individualized management plans, deliver clinical instruction, and empower patients to manage diabetes and related conditions.
You will work with primary care teams, document progress in the EHR, and stay current with diabetes technologies and guidelines while connecting patients to community resources and outreach initiatives.
