Job description
Corewell Health in Grand Rapids seeks a Diabetes Educator to provide education, counseling, and support to patients and families managing diabetes. You will document education and clinical recommendations, and measure outcomes to improve continuity of care across settings.
You will collaborate with other health professionals, contribute to program materials and policies, and help develop educational programs that advance quality services for the institution and community.
