Job description
Advocate Health Port Washington Clinic is seeking a full-time Diabetes Educator/Registered Dietitian to deliver DSMES per ADA/ADCES standards, assess learning needs, and lead individual and group sessions. If RD, medical nutrition therapy is provided; collaboration with the care team is essential.
The role requires RN or RD licensure; CDCES certification is preferred with a 2-year (up to 3 years) pathway if not certified.
