Job description
Beth Israel Lahey Health Primary Care in Dedham, MA is seeking a Diabetes Educator Team Nurse to join our multidisciplinary care team. The role includes coordinating outpatient diabetes education, performing annual wellness and chronic care management, and educating patients on self-management and diabetes technologies.
The ideal candidate is a registered or licensed practical nurse with CDCES certification, strong communication skills, and 1–3 years of relevant experience.
