Rochester Regional Health Home Care seeks a Certified Diabetes Educator to lead diabetes program development, implementation, and staff education for high-risk patients in home care settings.
The role involves policy development, staff training, and quality improvement activities. Requires active NY RN license and a Diabetes Educator Certification (or ability to obtain within 6 months). Travel within NYS may be required.
