GREATER NEW BEDFORD COMMUNITY HEALTH CENTER is seeking a Chronic Disease Nurse Educator. This RN collaborates with primary care providers to improve diabetes and hypertension outcomes, delivering education, monitoring, and medication administration under standing orders.
The Nurse Educator reviews data, titrates meds per protocols, orders labs as allowed, and communicates recommendations to the primary care team. Documentation in the EHR is essential to patient care and quality initiatives.
