Job description
Fresenius Medical Care Milwaukee is seeking a Transitional Care Unit (TCU) Registered Nurse to support patients starting dialysis. You will coordinate care and education to help patients adjust to their treatment choices—center-based or home dialysis—and work closely with the End Stage Kidney Disease care team.
The role includes leadership duties, evidence-based practice, and adherence to policy, with emphasis on safe, comprehensive patient care and education.
