Individual Support Counselors implements individualized programs as specified in Individual Service Plans, provides for individuals' health and welfare, and ensures general cleanliness and safety in individuals' homes.Providing Individual community outingsEncourage rapport with local resourcesInstruct and assist with meal preparationAssistance with activities of daily livingInstruct and physically assist with housekeeping activitiesMedical appointmentsAdminister and documentation of medication and/or monitor medical healthAssist with financial managementEnsure home is safeWhat are Community Outings?As a Individual Support provider you will provide opportunities for individuals to integrate into the community and enjoy outings that interest them.
