Direct Service Worker (DSW) - Jennings, LA
Location: Jennings, LA 70546
Pay: $12.00 per hour
Job Type: Part-time
Requirements: Must have a valid Driver’s License, valid driving documents & reliable transportation.
Days: Monday through Friday
Hours: Up to 40 total hours weekly

The candidate MUST have:
A valid driver’s license
Valid driving documents
Reliable transportation
The ability to successfully pass all state-mandated background checks and screenings
Shift Details: Direct Support Professional (Caregiver)
Monday: 8:30 AM–4:30 PM – Shared Support
Tuesday: 8:30 AM–4:30 PM – Individual Support
Wednesday: 8:30 AM–4:30 PM – Shared Support
Thursday: 8:30 AM–4:30 PM – Individual Support
Friday: 8:30 AM–4:30 PM – Shared Support
Total Schedule: Up to 40 hours per week
Core Responsibilities: Direct Support Professional (Caregiver)
Personal Care: Assist consumers with daily routines and personal care needs as outlined in their individual service plans.
Home Support: Assist with meal preparation, light housekeeping, and maintaining a safe and comfortable living environment.
Daily Activities: Provide support with daily routines and activities while encouraging independence and personal choice.
Community Support: Assist with community activities and appointments when required.
Daily Documentation: Maintain accurate documentation of services provided and communicate any concerns or changes according to SLIC procedures.
Requirements: Direct Support Professional (Caregiver)
Age: Must be 18 years of age or older.
Documents: Valid Driver’s License or Valid ID and Social Security Card
Background: Must pass all state-mandated background checks and screenings.
Transport: Reliable transportation to consumer homes in Jennings may be required depending on specific position.
Character: A patient, compassionate, and reliable work ethic.
