Stabilization Home Services Independent Living CoachUnder the supervision of the Residential Supervisor, the Stabilization Home Services Independent Living Coach provides supervision, guidance, and support to people served by Trinity Services. The Independent Living Coach helps create an environment that promotes independence, personal choice, skill development, and achievement of individual outcomes. All supports and services are provided in a dignified, respectful, person-centered, and professional manner.Salary: $23.05–$23.75 per hour Location: Elwood, IL Reports To: Residential Supervisor Network/Department: Network 4 Classification: Non-ExemptEssential FunctionsPromote a safe, supportive, and respectful living environment that protects the dignity and rights of the people served.Advocate for the rights, choices, preferences, and needs of individuals receiving services.Interact with people served in a supportive, encouraging, and professional manner.Provide assistance to maintain a safe and healthy living environment.Encourage and support individuals in making choices and developing greater independence.Support individuals in developing and maintaining friendships and positive relationships.Assist individuals with planned leisure and recreational activities that are appropriate for their age and interests.Follow approved medication administration procedures and documentation requirements, as applicable.Monitor and report health concerns and ensure individuals receive appropriate medical attention.Provide transportation to employment, medical appointments, shopping, community activities, and other personal needs, as assigned.Maintain assigned vehicles in a safe and clean condition.Respect and maintain the privacy and confidentiality of people served.Follow and implement each individual's Individual Service Plan (ISP).Support individuals with daily living skills, including personal hygiene and grooming, laundry, room cleaning, meal preparation, and other household tasks.Assist individuals in accessing community activities, services, and events.Support appropriate daily routines, including waking and bedtime routines, bathing, meals, and appropriate clothing and attire.Participate as a member of the Stabilization Home Services team in the development, implementation, and review of Individual Service Plans.Maintain accurate daily documentation and communicate relevant information to other staff members.Utilize approved verbal de-escalation and physical intervention techniques when necessary and in accordance with agency training and policy.Successfully complete required agency training upon hire, including CPR, First Aid, and Safety-Care, as applicable to the position.Successfully complete the NADD DSP certification exam within 60 days of hire.Perform other duties as assigned.Skills And AbilitiesAbility to establish and maintain effective working relationships with people served, coworkers, supervisors, and other professionals.Ability to maintain a professional demeanor during all interactions.Ability to communicate effectively both verbally and in writing.Ability to accurately and efficiently document services and activities.Strong time-management skills, including the ability to report to scheduled shifts on time.Ability to follow written and verbal instructions.Ability to work independently and as part of a team.Ability to safely perform physical tasks required to support individuals with daily activities.Ability to bend, lift, carry, push, pull, stand, reach, stoop, kneel, crouch, and perform repetitive movements as required by the position.Ability to safely perform approved physical interventions when properly trained.Ability to operate a motor vehicle safely.Physical DemandsThe physical demands of this position may include:Standing and sitting for extended periods.Walking, balancing, climbing, reaching, grasping, pushing, pulling, lifting, stooping, crouching, and kneeling as necessary to assist individuals with daily activities.Pushing wheelchairs and assisting individuals with mobility needs.Lifting and carrying items weighing up to 40 pounds.Pushing and pulling objects or equipment as required.Operating a motor vehicle.Performing approved physical intervention techniques when required and properly trained.Using a computer, telephone, and other standard office or communication equipment.QualificationsAt least 1 year of previous Direct Support Professional (DSP) experience working with individuals with intellectual/developmental disabilities or in the mental health field.High school diploma or GED required; bachelor's degree preferred.Valid driver's license required.Ability to successfully complete required pre-employment assessments, including a reading comprehension assessment.BenefitsHealth, vision, dental, and life insuranceEducational assistancePaid time offPaid holidaysSick timePersonal timePotential year-end bonusSign-on bonusFinancial Assistance Program (Cookie Jar)Employee Referral Program (Bounty Bucks)Benefits may vary based on employment status and eligibility.
