Services are provided in the individual's home and throughout their community, aligned with person-centered goals and outcomes.Key ResponsibilitiesSupport individuals in achieving goals through person-centered approaches.Maintain a balance between health and safety and individual independence.Assist with home and community-based activities that promote growth and engagement.Maintain professional relationships with families and team members.Attend team meetings and communicate regularly with supervisors.Complete all required trainings and comply with incident reporting standards.Use a smart device for documentation and communication as needed.QualificationsHigh School Diploma or GEDAbility to complete one-person physical transfersValid Pennsylvania driver's licenseMust meet Act 33/34 clearance requirements Why Join Achieva?
