Job Duties:
Assist individuals with personal care and self-help skills
Support transportation and community-access needs
Prepare meals and provide dining assistance
Maintain a safe, clean, and comfortable living environment
Monitor individuals' overall health, safety, and well-being
Encourage independence and participation in daily activities
Provide support within intermediate care facilities, community training homes, and supervised living programs
Document services and daily activities in an electronic record-management system
Communicate professionally with individuals, coworkers, and support teams
Requirements:
Compassionate, dependable, and patient approach
Strong verbal and written communication skills
Ability to provide personal and residential care
Basic computer skills for electronic documentation
Ability to follow individualized support and safety plans
Strong attention to health, safety, and accountability
Ability to work independently and as part of a care team
