Job description
Primary PartnerCare in Great Neck, NY is seeking a Head, Care Management & Clinical Programs/Innovation (Medicare patients) for a full-time on-site role, traveling to practice sites across Nassau, Suffolk and Queens.
You will lead care management and clinical innovation initiatives, develop and scale programs, ensuring regulatory compliance, and deliver patient-centered solutions to improve Medicare outcomes.
