Job description

Director Case Management - Step into a high-impact leadership role where innovation meets compassion and elevate your career as a transformational Director Care Management guiding exceptional patient outcomes across a dynamic acute care environment.

The Director Case Management will join a mission-driven healthcare organization known for clinical excellence, patient-first outcomes, and a culture of accountability and safety. This respected community-based hospital network has earned national recognition for nursing excellence and consistently achieves strong quality metrics, patient satisfaction scores, and low hospital-acquired infection rates. Leaders in this organization benefit from executive visibility, data-driven decision-making tools, modernized technology platforms, and an environment that supports interdisciplinary collaboration across complex medical and surgical service lines. With a reputation for stability, strategic growth, and measurable community impact, the Director Utilization Review will play a pivotal role in advancing value-based care initiatives and optimizing patient throughput across the continuum.

The Director Case Management will oversee a comprehensive Case Management and Utilization Review department within a 240+ bed not-for-profit community hospital network offering a distinct combination of nationally recognized clinical quality and deeply localized care across the Hudson Valley. The hospital holds the prestigious Magnet Recognition Program® status from the American Nurses Credentialing Center. Reporting on performance metrics related to length of stay, readmissions, denials management, and transitions of care, the Director Utilization Review will lead evidence-based resource management strategies while ensuring regulatory compliance and payer alignment. This department serves as a critical bridge between clinical teams, patients, families, and post-acute partners to ensure seamless, efficient, and patient-centered care coordination.

The Director Care Management will report directly to a senior hospital executive and provide strategic and operational leadership to nurse case managers, social workers, utilization review specialists, and support staff. The Director Case Management will mentor an experienced and collaborative team of compassionate nurses and diverse healthcare professionals dedicated to advocacy, discharge planning excellence, and interdisciplinary teamwork. Responsibilities include departmental budgeting, policy development, quality improvement initiatives, staff development, physician collaboration, and strengthening relationships with payers and community agencies. Strong executive partnership, accessible leadership, and engaged frontline clinicians create a supportive infrastructure for success.

Located in the scenic Hudson Valley New York region, the Director Case Management will enjoy a lifestyle that blends natural beauty with vibrant culture. The area offers breathtaking river views, hiking trails, biking paths, farmers markets, wineries, and four-season outdoor recreation. Residents appreciate charming historic towns, excellent schools, diverse dining options, thriving arts and music festivals, and convenient access to major metropolitan amenities without the congestion of city living. The Hudson Valley features a strong local economy, welcoming neighborhoods, reliable transportation options, and proximity to higher education institutions, making it an ideal setting for professionals and families seeking balance, opportunity, and quality of life.

The Director Utilization Review will be offered competitive executive-level compensation along with a comprehensive suite of health, dental, and vision insurance, retirement plans with employer contributions, paid time off, tuition support, and professional development resources. This is an exceptional opportunity for an experienced healthcare leader ready to make a measurable impact in Case Management leadership.
