Job description
HealthPoint seeks a Director of Medical Services, Population Health & Clinical Informatics to partner with the CMO and lead provider operations across 11 clinics in eight counties. You will oversee clinical workflows, mentoring of APPs, and alignment of population health and informatics initiatives to improve care quality.
You will supervise APPs, support onboarding, and drive value-based care programs while interfacing with Finance, Compliance, and HIT to optimize performance across the network.
