Job description
Texas Health Resources in Fort Worth, TX seeks a Director of Care Transition to lead the daily operation and strategic development of the care transition function. The role directs Care Transition Managers (RN & SW) and partners with nurses, physicians, and payors to improve discharge planning, patient outcomes, and compliance.
The position requires a Bachelor's in Nursing or a Master's in Social Work, with 5 years of related hospital case management experience and active licensure.
