Job description
Leoforce in Sioux Falls, SD seeks a Director of Case Management for an inpatient rehab hospital. You will lead daily operations, oversee utilization review, care coordination, and discharge planning for a 40-bed unit.
The role requires an RN with BSN (or non-RN with bachelor’s) and CCM/ACM certification, plus 3 years hospital CM experience. You’ll build the team and drive discharge-score improvements with strong leadership.
