Jobot is seeking a Director of Case Management in Yuma, AZ to lead a growing Case Management team and drive patient care outcomes at a premier inpatient rehabilitation hospital.
You will supervise Case Managers, oversee discharge planning, collaborate with physicians and other disciplines, monitor patient progression, and promote safe, effective transitions home while supporting organizational growth.
