Job description
Essen Health Care is expanding its Director of Collaborative Care (CoCM) leadership to scale an integrated behavioral health program within primary care across its network in New York. The role reports to the SVP of Care Management and partners with CAO on program strategy and operations.
The Director will supervise staff, ensure NCQA/DOH/CMS compliance, optimize billing, and strengthen payer and community partnerships to sustain growth and quality outcomes.
