Rocky Mountain Care – Clearfield | Clearfield, Utah

Lead a Strong Team. Make Your Mark.

Experienced Directors of Nursing know the difference between walking into a building that needs to be rebuilt and walking into a building with scale, structure, experienced leadership, and a team you can lead.

This is the second one.

Rocky Mountain Care Clearfield is a 125-bed skilled nursing and long-term care facility with a current census of approximately 108 residents, including a ventilator/tracheostomy unit and a strong post-acute rehabilitation census.

We're looking for an experienced Director of Nursing (DON), Director of Nursing Services (DNS), Assistant Director of Nursing (ADON), Nurse Manager, or other experienced RN nursing leader who thrives in a fast-paced environment and is ready to lead a large nursing operation.

Why This DON Role Is Different

You won't be doing it alone.

Clearfield already has:

125 licensed beds

Approximately 108 current residents

Ventilator/tracheostomy unit

High post-acute rehabilitation census

Experienced Administrator

Established Unit Manager leadership structure

Strong CNA leadership structure

Experienced MDS support

A clinically complex, fast‑paced skilled nursing environment

You won't be walking into a building where you have to build the leadership infrastructure from the ground up.

You'll be stepping into a large operation with an experienced team already in place—and have the opportunity to make your mark.

Our Mission Is People.

At Rocky Mountain Care, our mission is People.

We believe great care starts with how we treat the people who provide it. Our culture is built around seeing people as people—valuing employees, residents, and families while creating an environment grounded in leadership, trust, respect, dignity, accountability, and teamwork.

As DON, you'll have a direct opportunity to bring that mission to life.

You'll lead people before you lead processes.

You'll develop your nursing leaders, build accountability, recognize great performance, strengthen teamwork, and create an environment where nurses and CNAs can do their best work.

For the right leader, this isn't simply an opportunity to manage a nursing department.

It's an opportunity to build a culture.

If you're an experienced DON, ADON, DNS, RN Director, Nurse Manager, or skilled nursing leader ready to lead a larger operation with an experienced leadership team around you—and you believe that People are the foundation of exceptional care,we'd like to talk with you.

Lead the team. Elevate the care. Put People first. Make your mark.
