Director of Nursing (DON) – Lead Home Health Clinical Operations

All Seniors Foundation is a 501(c)(3) non-profit committed to improving the quality of life for older adults across Los Angeles County. we provide medical care, in-home support, hospice and palliative care, cancer navigation, and 200+ other free services to qualifying seniors. Every service is delivered at no cost, regardless of insurance status. Role Overview:
As Director of Nursing, you will provide clinical leadership for our entire home health program — supervising nursing staff, ensuring CMS compliance, leading QAPI, and shaping clinical strategy. You report directly to the CEO and partner with the Medical Director on clinical quality. Key Responsibilities:
Provide clinical supervision and leadership to RNs, LVNs, HHAs, and therapy staff (40+ clinicians)
Ensure compliance with CMS Conditions of Participation, CA Title 22, and Joint Commission standards
Lead the QAPI program: chart audits, performance improvement, Star Ratings, HHCAHPS
Develop and update clinical policies, procedures, and competencies
Manage clinical hiring, onboarding, performance evaluation, and progressive discipline
Represent the agency in state surveys, accreditation reviews, and payer audits
Qualifications:
Current CA RN license + BSN required; MSN strongly preferred
5+ years home health nursing including 2+ years in management
Familiarity with OASIS, CMS regulations, Joint Commission, and CA DPH home health rules
Strong leadership, communication, and conflict‑resolution skills
COS-C, CHPN, or CPHQ certification a plus
