Job description
ClearSky Rehabilitation Hospital of Lakeshore, WI invites a Director of Patient Outcomes to lead case management, monitor patient outcomes, and drive compliance across CMS13 and medical necessity reviews.
Ideal candidates hold an active clinical license (PT/OT/SLP or RN preferred) with 2+ years in acute care, and are prepared to collaborate with HIM, operations, and physicians to ensure top-quality patient care. On-site locations include Eau Claire, Beloit, Casa Grande, Seneca, and Smithfield.
