Responsibilities
Oversee rehabilitation operations at multiple assigned facilities.
Supervise, mentor, and support facility-level DORs and therapy staff.
Provide treatment as appropriate to professional designation/licensure.
Ensure therapy services meet Innovate Rehab & Wellness standards, as well as federal, state, and payer regulations.
Monitor productivity, quality outcomes, and financial performance across facilities.
Partner with facility administrators and nursing leadership to address operational challenges.
Conduct site visits, audits, and compliance checks; implement corrective action when needed.
Support recruitment, onboarding, and professional development of therapy leaders.
Report area performance metrics to Area Director or VP of Operations.
Foster consistency in therapy programs, policies, and culture across assigned facilities.
Qualifications: Qualifications
Licensed PT, OT, SLP, PTA, or COTA/OTA.
Minimum 4–6 years of experience in long-term care rehabilitation, with at least 2 years in a leadership role.
Multi-site management experience preferred.
Strong knowledge of Medicare/Medicaid regulations and SNF compliance requirements.

Lift/carry laptop, files, or equipment up to 20–25 lbs.
