Job description
Evolve Therapy Services seeks a Director of Rehabilitation (PT, PTA, OT, OTA, SLP) for an in-house Skilled Nursing Facility located in Franklin, MN. You will oversee therapy operations, ensure compliance with regulatory standards, and collaborate with the facility team under the Regional Director of Operations.
Required licensure in relevant disciplines, PDPM/MDS familiarity, and strong leadership/communication skills to drive high-quality patient care. Sign-on bonus offers additional incentive.
