Job description
Evolve Therapy Services is seeking a Director of Rehabilitation (PT, PTA, OT, OTA, SLP) for an in-house Skilled Nursing Facility in Franklin, MN, with an Olivia, MN location as needed. A $5,000 sign-on bonus and salary range of $75,000–$110,000 are offered.
The role oversees therapy operations, ensures regulatory compliance, and collaborates with the facility team under the Regional Director of Operations. Licensure in applicable state(s) is required and PDPM/MDS knowledge is preferred.
