Evolve Therapy Services is seeking a Director of Rehabilitation for an in-house Skilled Nursing Facility in Freeport, IL. The role oversees the entire therapy program, ensuring care meets standards and aligns with company policies under regional leadership.
Candidates must hold a Degree in PT/OT/SLP, hold active licensure in applicable states, and maintain ongoing professional development. PDPM/MDS experience is preferred, with strong leadership and communication skills.
